Geriatric eToolkit

An intercollaborative initiative by the Centre for Studies in Aging & Health, the Geriatric eToolkit is an interprofessional resource developed tohelp direct care providers deliver comprehensive and expedited evidence-based geriatric care.

Grouped for various practice settings including primary care, emergency care, acute care,community care, specialized care and long-term care, the tools are organized into four categories:

  • Screening Tools - Simple screens that flag the need for further assessment
  • Assessment Tools - More detailed assessment of single issues
  • Comprehensive Assessment Tools - Thorough or holistic assessment of one or more issues
  • Decision Support Tools - Information supporting clinical decision-making with respect to assessment and management of geriatric issues

Bridges to Care Long-Term Care Tool Kits

Long-Term Care (LTC) Tool Kits have been developed as part of the Building Capacity for Inter-professional Collaborative Care in Long-Term Care (BCICC-LTC) “Bridges to Care” Project. There are four topic areas - Click on the topic to access the tool kits:
