Specialized Geriatrics Program in South East Ontario
Specialized Geriatrics provide specialized geriatric multidisciplinary services throughout South East Ontario. The catchment area of Specialized Geriatrics includes Hastings, Prince Edward, Lennox & Addington, Frontenac, Leeds & Grenville and parts of Lanark and Northumberland Counties.
Operated from Providence Care's St. Mary’s of the Lake Hospital site, these specialized geriatric services range from hospital to home. We seek to optimize the health of seniors in the region by working in collaboration with clients, families/caregivers, family physicians, community service providers and others.

Who may benefit from specialized geriatric care?
- Aged 65 years and older with complex health issues
- Recent decline in managing day-to-day activities
- Difficulty with getting around
- Recent falls or fear of falling
- Concerns about medications
- Recent changes in ability to cope
- Recent changes to social supports and/or caregiver stress
- Multiple admissions to hospital or visits to the emergency department or family physician
- Mood/memory/thinking changes as part of other health changes
- Recent and unexplained changes in health status
The goal of specialized geriatric care
Reduce the burden of disability, associated with frailty, by detecting and treating reversible conditions and recommending optimal management of chronic conditions.
The need of specialized geriatric care
Frail seniors with complex health problems have unique needs and present specific challenges for accurate diagnosis, assessment and treatment.

Clinical services
Assessment
- Outreach: Assessment within the client’s own home, whether that be in the community or an institutional setting.
- Acute Care Consult: Inpatient consultation, assessment, care planning and recommendations at Kingston General Hospital and Hotel Dieu Hospital.
- Clinics: Specialized geriatric assessment, diagnostics and treatment. Clinics include: Geriatric Medicine, Psychology, Continence and Multidisciplinary.
Rehabilitation
- Day Hospital: half-day outpatient assessment and rehabilitation at St. Mary’s of the Lake Hospital.
- Inpatient Unit: 16-bed unit for assessment, short-term rehabilitation and treatment at St. Mary’s of the Lake Hospital.
Non-clinical activities
Research and Evaluation
Care providers link with evaluators and researchers to identify and promote evidence-based, high quality care and enhanced processes.
Education and Capacity Building
We enhance knowledge exchange through education, training and mentoring within academic settings and in community practices.
Health System Integration
We link with key regional and provincial networks, seniors, caregivers and care providers to assist with access and navigation through services and resources.
Health Service Innovation
We support primary care reform, family health teams, chronic disease management, elder-friendly hospitals, integrated service plans and shared care models.
