Clinical services are available in four sites in Kingston
Kingston General Hospital - In-Patient Consultation Service
When a patient is in the hospital, the team most responsible for their care may ask the QPCMP team to help them with specific palliative care needs. The QPCMP team does not take over patient care but works with the team most responsible for the patient's care. A patient (and/or their families, friends & caregivers) may ask their family doctor to refer them to the Queen’s University Palliative Care Medicine Program for symptom management concerns. Family doctors can access this resource 24 hours a day, 7 days a week.
Cancer Centre of Southeastern Ontario - Outpatient Palliative Medicine Clinics
The patient's family doctor or cancer doctor can refer the patient to one of the Palliative Medicine Clinics at the Cancer Centre of Southeastern Ontario for help with symptom management and end-of-life issues. The QPCMP team in the Cancer Centre will follow the patient's care along with their family doctor and/or cancer doctor.
Community (Greater Kingston Area)
Within the Greater Kingston Area, QPCMP doctors are available to help family doctors care for patients in the community setting (home, residential and nursing homes) if they have difficult to manage palliative care needs. The family doctor can request that the QPCMP doctor shares patient care with them, or that the QPCMP doctor takes over patient care completely. If a patient has no family doctor, their cancer doctor or other specialist can make this referral.
Providence Care - St. Mary’s of the Lake Hospital - Palliative Care Unit (PCU)
Six palliative care beds are located at St. Mary’s of the Lake Hospital for patients from the Greater Kingston Area nearing the end of life who have too many needs to be cared for in the community. A patient's family doctor can make a referral to the Palliative Care Unit (PCU). If the patient has no family doctor, their cancer doctor or specialist make this referral. The QPCMP team at the PCU takes over patient care completely when a patient is admitted.
