Palliative Care Services in South East Ontario

There are several ways to get Palliative Care Services for people living in South East Ontario. Service providers are listed by location under two categories: Hospital-based Palliative Care Services and Community-based Palliative Care Services.

Most referrals must be made by a family doctor or a specialist. However, a patient and/or their families, friends or caregivers may also request a referral to receive palliative care services. Palliative care services are available in the following locations:

Hospital-based Palliative Care Services

Queen’s Palliative Care Medicine Program

A patient (and/or their families, friends & caregivers) may ask their family doctor to refer them to the Queen’s Palliative Care Medicine Program for symptom management concerns. Family doctors can access this resource 24 hours a day, 7 days a week by calling (613) 548-2485.

Palliative Care Advance Practice Nurse at Kingston General Hospital

For patients already being treated at Kingston General Hospital. A referral to the Advance Practice Nurse can be made directly by a family physician or health care professional for help with decision-making, family and caregiver support while the patient is in the hospital. A physician or nurse in the hospital can put you in contact with the Advance Practice Nurse.

Cancer Centre of Southeastern Ontario

A family doctor or a cancer doctor can refer a patient to one of the Palliative Care Clinics at the Cancer Centre for symptom management. For more information about the Palliative Care Clinics, click here and click on the Programs and Services area.

Community-based Palliative Care Services

South East Community Care Access Centre (CCAC)

The CCAC arranges in-home and other community support services for people in Kingston, Frontenac, Lennox and Addington. A patient and/or their families, friends or caregivers can make a self-referral to the CCAC. A family doctor or the doctor most responsible for a patient's care will be notified of the CCAC referral.

Hospice Kingston

Hospice Kingston helps palliative patients and their families in Kingston and Frontenac by integrating compassionate care at home, in hospital or in nursing homes and meeting physical, emotional and spiritual needs. A patient and/or their families, friends or caregivers can make a self-referral to Hospice Kingston. Psychological and emotional support, visiting support, bereavement support and transportation services are available.

Southeastern Ontario Palliative Pain & Symptom Management Consultation Team (PPSMCS)

Funded by the Government of Ontario, and working out of CCAC offices throughout the South East LHIN, PPSMCS employs nurses with expertise in palliative care to provide information or bedside consultation to assist front line providers who care for people living with a life threatening illness.

Palliative Care Coordinator, Quinte Health Care

Quinte Health Care operates three hospital sites (Bancroft, Belleville, Trenton) in Hastings County. The Palliative Care Coordinator (based at Belleville) accepts referrals from health professionals, patients and/or their families, friends or caregivers from Monday to Friday. Support is offered for pain & symptom management, decision-making and other palliative care issues of importance.

Heart of Hastings Hospice is committed to improving the quality of life of people in Madoc, Marmora, Tweed, Stirling & surrounding rural areas who are facing the diagnosis of any life-threatening illness. A patient and/or their families, friends or caregivers can make a self-referral to Heart of Hastings Hospice. Psychological and emotional support, visiting support, bereavement support and caregiver respite services are available.

Hospice North Hastings

Hospice North Hastings assists palliative care patients and their caregivers in Bancroft & the surrounding areas by giving them support and companionship during their illness. A patient and/or their families, friends or caregivers can make a self-referral to Hospice North Hastings. Psychological and emotional support, visiting support, bereavement support and caregiver respite services are available.

Hospice Quinte

Hospice Quinte assists palliative care patients and their caregivers in Quinte by giving them support and companionship during their illness. A patient and/or their families, friends or caregivers can make a self-referral to Hospice Quinte. Psychological and emotional support, visiting support, bereavement support and caregiver respite services are available.

Perth & Smiths Falls District Hospital

Pain & symptom management, physical, social, psychological, spiritual and bereavement support for palliative patients and their families at two locations (Perth and Smiths Falls).

Community-based Palliative Care Services

The Volunteer Hospice Visiting Service of Community Home Support Lanark County assists palliative care patients and their caregivers in Leeds & Grenville Counties by giving them support and companionship during their illness.
