Palliative Care Integration Project (PCIP) Resource Manual
Developed by the the Kingston, Frontenac, Lennox and Addington Palliative Care Integration Project (PCIP), the Palliative Care Integration Project (PCIP) Resource Manual is a binder compliation of all clinical materials associated with the Project.
Colour-coded, and organized for printing, the Resource Manual contains all of the Palliative Symptom Assessment Tools, Collaborative Care Plans, Symptom Management Guidelines, Documentation and Referral Information associated with the Project.
NOTE: The Symptom Management Guidelines are password protected and available for use to practitioners that have received education about PCIP and the management symptoms experienced by palliative patients.
