MICT (Mobile Interprofessional Coaching Team) II Project


Introduction

Phase I of the MICT (Mobile Interprofessional Coaching Team) initiative was successful in building a strong foundation for interprofessional care/education via face-to-face collaboration.

Phase II of the initiative (MICT II) aimed to create systems, processes and tools to foster a consistent, learner-driven and sustainable collaborative interprofessional enterprise.

Project Objectives

  • Create a practice environment within participating organizations and broader communities of practice that support interprofessional care and education initiatives across primary care, geriatric mental health, and the community sector to benefit the care of older adults with complex, co-morbid and often chronic mental and physical health needs
  • Build the capacity (i.e., knowledge, skills experience) of post-licensure professionals to engage in and benefit from effective interprofessional care practices with providers in other key disciplines and sectors
  • Foster role and responsibility sharing between disciplines, organizations and sectors within the participating regions to benefit older adults with complex mental and physical health needs
  • Link pre- and post-licensure professionals in practice and education forums to foster a common vision, language & approach for the target population
  • Utilize e-health technology to create a broad and sustainable mechanism for effective and efficient collaboration across primary care, geriatric mental health, and community sectors in the two participating regions
  • Enhance access to pertinent materials (toolkits, fact sheets, articles, question & answer forums, etc.) relevant to the care of older adults with complex mental and physical heath needs

The MICT II initiative aimed to contribute to overall health promotion in the following areas:

  1. Improved access for older adults with complex, co-morbid and chronic mental and physical heath needs to effective an efficient healthcare
  2. Improved access for primary care providers to pertinent specialty knowledge, skills, experience and resources relevant to the care of older adults with complex, co-morbid and chronic mental and physical heath needs
  3. Improved outcomes for older adults with complex, co-morbid and chronic mental and physical health needs
  4. Increased compatibility of post-licensure providers in primary care, geriatric mental health, and the community sector with a transformed healthcare agenda
  5. Establishment of a common vision, language and approach to the care of older adults with complex, co-morbid mental and physical health needs across the practice-education continuum

Project Strategies

The project strategies for MICT II were:

  • Promote a consistent, ongoing culture of collaboration by providing access to materials and discussions between collaborative sessions developed in MICT Phase I
  • Enhance “just-in-time” learning through an interactive discussion forum
  • Encourage a learner-driven process
  • Increase interactivity between Family Health Team sites and decrease dependency on content specialists
  • Decrease primary care reliance upon extended resources
