Eldercare Access Strategy in Emergency Rooms [easier+]

An integrated service plan for 2007-08 was developed as part of a strategy to alleviate pressures within Emergency Rooms (ER) at Quinte Health Care - Belleville General Hospital and Kingston General Hospital.
Eldercare Access Strategy in Emergency Rooms [easier+] included an integrated, multi-component strategy responding to three targeted priorities (increased service communication/ collaboration; enhanced ER capacity; and improved outcomes) in providing effective client-centred care to the high risk, frail elderly accessing the ER in South East Ontario.
Eldercare Access Strategy in Emergency Rooms [easier+] was made possible by one time funding in 2007-2008 to Providence Care, SE Community Care Access Centre and Community Support Services (CSS) Programs.
