The Ontario Gerontology Association held their 31st Annual Conference in Toronto, Ontario on April 25-26, 2012. The Centre for Studies in Aging & Health at Providence Care conducted a workshop at this event to share knowledge about:
Creating Future Leaders in Eldercare - Phase 2
Overview
For this project, the Centre for Studies in Aging & Health at Providence Care facilitated geriatric medicine and psychiatry interprofessional staff in leading QI projects which improved:
i) continuity of care;
ii) safe medication administration;
iii) safe discharge processes; and
iv) enhanced quality eldercare.
