Community Health Centres in South East Ontario
Community Health Centres (CHC's) are non-profit organizations that provide primary health and health promotion programs for individuals, families and communities. A health centre is established and governed by a community-elected board of directors. Community Health Centres provide education and advice on helping families access the resources they need from other community agencies.

Belleville - Belleville and Quinte West Community Health Centre
The Belleville and Quinte West Community Health Centre is a not-for-profit health care organization that provides a wide range of programs and services to improve the health and well-being of people who live in the community.
The Centre operates in two locations: one in Belleville and the other in Quinte West/Trenton. Services are available to everyone. Young families, children and single mothers, youth, seniors, people living in poverty and those who sometimes have difficulty getting health care are especially welcome. Their programs and services include primary health care, health and wellness promotion, and disease and illness prevention.

Kingston - Kingston Community Health Centres
The Kingston Community Health Centres (KCHC) share a common philosophical premise and mission statement. They care for individuals and families, respond to their neighborhood concerns to improve their health and to build healthy communities. In conjunction with the Smiths Falls Community Health Centre, the Merrickville District Community Health Centre offers a variety of primary health care, health education and health promotion services using a team approach. Portland - Country Roads Community Health Centre
The Country Roads Community Health Centre provides primary health care services and health promotion programs to North Leeds area residents. Country Roads Community Health Centre's multi-interdisciplinary team includes physicians, nurse practitioners, nurses, a dietitian, a chiropodist, social workers, a community health worker and health promoters. Amongst other programs, Country Roads provides chronic disease prevention and management programs including diabetes clinics, asthma/lung health programs and self management groups.

Tweed - Gateway Community Health Centre
The Gateway Community Health Centre (GCHC) is a non-profit, community-governed health organization serving the Municipality of Tweed. GCHC is committed to a wellness approach to primary health care delivered by a health care team comprised of Physicians, Registered Health Professionals including Nurse Practitioners, Nutritionist and Social Worker.
