Community Care Access Centres

Established by the Ontario Ministry of Health and Long-term Care, Community Care Access Centres (CCACs) are local agencies that provide access to government-funded home and community services, and long-term care homes.


Community Care Access Centre Services for Seniors

Services that Community Care Access Centres (CCACs) provide to seniors include:

a) Identifying care needs for seniors
b) Exploring the options best suited to each senior’s needs and personal situation
c) Determining a senior’s eligibility for government-funded services and settings
d) Gathering information about providers (home, community support services and residential care) in the surrounding area
e) Finding out how services are delivered
f) Arranging for delivery of government-funded home and community support services
g) Arranging and applying for admission to a long-term care home
h) Getting on the appropriate waiting lists

Launched in December 2012 by the South East Community Care Access Centre, SouthEasthealthline.ca is an innovative web site that puts accurate and up-to-date information about health services at the fingertips of consumers and health care providers across the South East region of Ontario. SouthEasthealthline.ca provides easy access to a reliable source of data, and empowers consumers to find the services they need close to home.

Over 4,000 service listings describe organizations and programs serving people who live in Leeds, south Grenville, south Lanark, Frontenac, Kingston and the Islands, Lennox and Addington, Hastings, Brighton, and Prince Edward County. Users can choose to receive local information first or search the entire South East region.
