Focus on the care of the elderly

Based in Kingston, Ontario, Canada, sagelink.ca provides on-line access to information, links, educational material and resources related to the care of the elderly with complex mental and physical health needs.

In conjunction with various continuing education programs for health professionals in South East Ontario, sagelink.ca also provides access to evidence-based clinical resources (toolkits, decision frameworks, fact sheets, best practices guidelines, etc.) targeted at issues and conditions that are specific to older adults.

Navigating sagelink.ca

Users can navigate sagelink.ca in several ways:


Topic Navigation List

The main Topic Navigation List is displayed in the left hand column of every page of sagelink.ca. Simply click on the topic displayed in column which will take you to that category, and open up sub-category pages.

Dropdown/Flyout Menus

Across the top of each page are icons depicting the five main categories of sagelink.ca. These are Dropdown/Flyout Menus which provide access to the first two levels of the Topic Navigation List when you move your mouse over the various categories.

Breadcrumbs

Breadcrumbs are a series of links that take you through the pages you have visited to get to your current page. The 'Breadcrumbs' are located immediately under the Dropdown/Flyout Menus on every page of sagelink.ca.

Site Map

We provide a Site Map which provides links to all our material and is located on the bottom of every page. The Site Map page also has a list of pages associated with certain Search Terms, including: Aging-Related Topics, Health Care Services in South East Ontario, Type of Care Setting, and Type of Resource.

Content layout

A main part of the new design of sagelink.ca is the new organization of content on the web site in comparison with the previous version. The web site is now is organized into five main categories:


FASTlinksBETA

FASTlinks are web-based maps that display links for elder care services in South East Ontario organized into 10 Common Elder Care Domains. These 10 Domains are organized by function, rather than disease diagnosis. FASTlinks have been developed as a system navigation tool for primary care health professionals in South East Ontario who are treating frail elderly patients with multiple co-morbidities (two or more health conditions that are unrelated but interact to affect the person’s overall health) associated with functional impairment. Patient & Caregiver Resources

Information, resources and links about the care of the elderly for seniors, their families, friends, caregivers and volunteers, organized into: Common Geriatric Issues, Other Medical Issues, Advocacy Organizations, and Support Groups.

Clinical Resources

CSAH, in collaboration with various partners, have created, adapted and translated several knowledge-to-practice clinical resources for health professionals relating to the care of the elderly that are suitable for a variety of care settings, organized into: Health Care Services (in South East Ontario), Clinical Topics, Long-Term Care Clinical Resources, Primary Care Clinical Resources, Palliative Care Clinical Resources, Toolkits and Best Practices Guidelines.

Education & Knowledge-to-Practice

Access to teaching resources about the care of the elderly for faculty, staff, teaching assistants and independent trainers, organized into: Teaching Resources, Learner-Focused Resources, Aging-Related Topics, Knowledge Exchange, and Learning Events & Workshops.

What population does sagelink.ca serve?

Sagelink.ca targets health care professionals and the public using the health care system in South East Ontario. South East Ontario covers a broad area that stretches north along Lake Ontario and the western section of the St. Lawrence River.

South East Ontario has an older population than the provincial average with higher percentages of the population in the age groups above age 45. New advancements in treatments, medications and technologies make it increasingly possible for elderly patients to be treated in a variety of settings. More and more in rural areas like South East Ontario, family physicians and the primary care community of health care professionals are principally responsible for managing the complex medical needs of this growing population. Largely a rural region, South East Ontario covers a broad area with a large number of small towns and villages.

Spanning over 20,000 square kilometres with a population of nearly 500,000, South East Ontario includes Frontenac County, Hastings County, Lanark County, Lennox & Addington County, Prince Edward County and the United Counties of Leeds & Grenville.

Cities and towns in South East Ontario include Bancroft, Belleville, Brockville, Gananoque, Kingston, Madoc, Napanee, Perth, Picton (Municipality of Prince Edward County), PrescottSmiths Falls and Trenton (City of Quinte West).
