As Ontario's population ages and medical science advances, a larger proportion of people will live with and die from chronic debilitating illnesses.
Southeastern Ontario has an older population than the provincial average with higher percentages of the population in the age groups above age 45. Largely a rural region, Southeastern Ontario covers a broad area with a large number of small towns and villages.
New advancements in treatments, medications and technologies make it increasingly possible for elderly patients to be treated in a variety of settings. More and more in rural areas like Southeastern Ontario, family physicians and the primary care community of health care professionals are principally responsible for managing the complex medical needs of this growing population.
sagelink.ca helps to effectively link primary care systems to specialized services which can significantly reduce undesired outcomes for elderly patients and their families by: - fostering constructive and interactive exchange of knowledge and best practices;
