There are a few ways to find family physicians and other options for primary health care in Southeastern Ontario.

Health Care Connect
Health Care Connect is a program launched by the Ontario Ministry of Health and Long-Term Care to help Ontarians without a family health care provider find one. The program refers people without a regular family health care provider to physicians and nurse practitioners who are accepting new patients in their community.

After Hours Clinics
After Hours (or Walk-In) Clinics provide Non-Emergency Family Health Care Services after normal week-day work hours and in some cases on weekends.

Family Health Teams
In Ontario, Family Health Teams are groups of doctors, nurses, nurse practitioners, nutritionists and other health care professionals who work together to provide primary health care to the public.

Community Health Centres
Community Health Centres (CHCs) are non-profit organizations that provide primary health and health promotion programs for individuals, families and communities. CHCs provide education and advice on helping families access the resources they need from other community agencies.
