Preparing for a Move Into Long-Term Care Produced in collaboration by the KFL&A Dementia Network, Preparing for a Move Into Long Term Care; "A Checklist" has been developed to help family members plan the move of a family member into a Long-Term Care Home. To quote from the introduction: "The application for long-term care has been completed. This move will be a major change in your family member’s surroundings. The following Checklist will help you plan the next steps to take in moving your family member to their new home. The checklist provides you with a number of practical tips and things to consider to help you prepare." The document is divided into five sections: 1. While waiting for the bed offer. 2. The call comes and you accept the offer. 3. The day of the move. 4. Questions you can expect to be asked. 5. In the days and weeks following the move.
