South East Ontario Regional Dementia Network

Dementia Networks are designed to improve the system of care for individuals with Alzheimer Disease and Related Dementias (ADRD), their families and caregivers by creating networks of service providers, clinicians, caregivers, agencies, and health related organizations.

The mandate of the South East Ontario Regional Dementia Network is to act as an agent to support the development and evolution of the local Dementia Networks of Hastings and Prince Edward, Kingston, Frontenac, Lennox and Addington and Lanark, Leeds and Grenville Counties.

Dementia Care Project

The Dementia Care Project was commissioned by the South East Ontario Regional Dementia Network to gather current data on services and needs of persons with Alzheimer Dementia & Related Dementias (ADRD) and their caregivers, to identify challenges and priorities, and outline key recommendations to guide the future direction of the network in meeting the identified needs of ADRD clients.

Alzheimer Knowledge Exchange (AKE) Resource Centre

The Alzheimer Knowledge Exchange (AKE) is a Community of Practice of people dedicated to improving the quality of life for persons with Alzheimer Disease and Related Dementia (ADRD). The purpose of the AKE is to promote and support a knowledge exchange interface amongst researchers, educators, care providers (paid and non-paid), policy makers and stakeholder organizations working in the field of ADRD.

Alzheimer Societies in South East Ontario

The Alzheimer Society is a non-profit organization and registered charity that offers support, education, and advocacy for families of people with Alzheimer’s disease and related dementias. The Society also focuses on raising money to help find a cure and cause for the disease. Offices are currently staffed in Bancroft, Belleville, Brockville, Kingston, Perth, and Picton.

Community Support Services in Southeastern Ontario

Community Support Services are provided by local, not-for-profit agencies. Such agencies co-ordinate the effort of many caring and committed volunteers. These support services are flexible, personalized, responsive and least intrusive. The services provided include a broad range of practical support services designed to help the elderly and adults with physical disabilities accomplish the tasks of everyday living.

Providence Care - Specialized Geriatrics

The Specialized Geriatrics Program, St. Mary’s of the Lake Hospital, and the Division of Geriatric Medicine are a regional resource in four areas: specialized clinical services, community development, education, research and evaluation. Clinical services include a 16-bed inpatient unit, day hospital, and outpatient clinics at St. Mary’s of the Lake Hospital. Acute care consultation and outreach assessments are provided in people’s homes, long-term care facilities, and hospitals throughout Southeastern Ontario.
